Hospitals Used AI To Inflate Medical Bills By Nearly $1 trillion Report Finds

"Hospitals’ use of artificial intelligence tools hiked healthcare spending by nearly $1 billion over a two-year period, per a Blue Cross Blue Shield Association analysis released Thursday.

BCBSA’s new study shows that a major rise in patients being documented as having complex conditions led to $942 million in additional medical care costs for Blue Cross and Blue Shield (BCBS) companies between 2023 and ‌2025. ..."

"Key Findings:
  • A sharp increase in patients being documented as having complex conditions appears to have added an estimated $942 million in healthcare spending for BCBS companies between 2023 and 2025, driving higher premiums and out-of-pocket costs for families, employers and taxpayers.
  • There is a clear disconnect between coding and treatment — our research shows that coding for care has changed, but there is no evidence of corresponding change in care delivered.
  • Patients often fell into a higher-cost billing category after a secondary diagnosis, a condition identified in addition to the patient's primary reason for receiving care. This secondary diagnosis may be derived from single laboratory values, making it particularly well-suited for detection by AI tools. Approximately 70% of the additional costs, more than $650 million, were tied to secondary diagnoses.
  • This growth in complex coding comes as more than 60% of hospital systems began using AI coding tools, which can document patient visits, analyze lab reports and doctors’ notes.

How Not Getting Sick Made the Economy Look Sick in terms of statistical data

Recommendable! Yes, medical services and more ill people increase GDP! A lesson in economics!

Economic textbooks have pointed out this paradox/negative self enforcing cycle for at least several decades that e.g. a roaring economy causes more employees to get stressed out, suffering more accidents etc. and thereby seeking more medical treatment driving up GDP further.

"... The Bureau of Economic Analysis (BEA) cut fourth-quarter real economic output growth to 0.7 percent from 1.4 percent. ...

Demand for health care was lower than initially estimated. The Census Bureau’s Quarterly Services Survey shows health care and social assistance revenue rose just 0.5 percent in the fourth quarter after a 3.0 percent gain in the third quarter. BEA specifically cited new QSS data as the reason it revised down health-care services.

Demand was down because we were healthier than expected. Data from the Centers for Disease Control show severe respiratory illness hospitalizations were low late in the fourth quarter, consistent with a lighter illness period than one would expect in a normal winter ramp-up. Fewer sick people means fewer hospital visits, fewer outpatient visits, and less measured health-care output. So a mild holiday season for the flu and COVID turns out to be bad for GDP accounting, but it is not bad news for actual Americans. ..."

More Hospitals/health systems Dropped Medicare Advantage in 2025 totalling 33/40

Bad news!

"Thirty-three hospital systems are dropping their Medicare Advantage (MA) contracts, reports Becker’s Hospital Review.

The publication has been monitoring participation rates for several years. In 2024, citing contracts ending in 2025, Becker’s lists 33 hospitals as becoming “out of network” for Medicare Advantage contracts.

In 2023, 13 hospitals ended their contracts.

In 2024, the publication identified 32 hospitals that ended MA participation. ..."

Nine pharmaceutical companies Makers to Reduce Prescription drug prices in the US for Medicaid recipients and cash buyers

I did not follow this story very closely. It appears that President Trump may have strong armed these companies in offering their drugs at lower prices in the US.

It has been argued for several decades that pharmaceutical companies around the world were taking advantage or exploiting the more open, free market of the US by offering higher priced drugs while accepting government price controls and numerous restrictions in other Western countries. As long as these companies were able to make large profits in the US they did not bother too much about limited business in other countries.

"The Trump administration announced new agreements on Friday with some of the world’s largest drug manufacturers aimed at lowering prescription drug costs for Americans. 

Nine pharmaceutical companies have agreed to reduce prices on their flagship drugs for Medicaid recipients and people who buy the medications in cash, rather than through insurance, as part of a new deal.

In addition, any newly launched drugs made by the companies will also be charged at the “most-favored-nation” pricing [or lowest price anywhere] across the country for everyone, including commercial and cash pay markets, as well as Medicare and Medicaid.

A senior administration official said about 30 to 40 percent of Medicaid drugs would be impacted."

Making Use of the huge VA Hospital Overcapacity: Expand Access to Reduce Costs

Food for thought! Will President Trump close or sell some of the excess VA hospitals?

I am not even sure it has been really necessary for the VA to run their own hospitals! How different are war injuries from e.g. shooting injuries in the US? To finance and support research and/or training of doctors for war injuries would probably suffice.

Once more are Americans perhaps overdiagnosed? Too many doctors, too many diagnoses?

This article contains the usual terms of art like "food deserts" or "low health literacy". A long litany of the usual suspects.

The United States has the greatest rate of people with multiple chronic conditions in spite of much higher rate of health care spending in comparison to other wealthy countries. In 2024, more than 23 percent of U.S. adults reported experiencing a mental illness—often considered a chronic disease that can worsen other chronic diseases. Mental illness can be amplified by other chronic conditions. ...

Although investments in preventative care in the United States have declined over time, the United States still spends more on disease prevention than other wealthy nations. ...

About nine out of 10 American adults have low health literacy—that is they lack the ability to understand health-related information. Low health literacy is associated with poor health outcomes. Boosting health literacy in the United States may require amplifying health education efforts in K—12 educational settings, through employers, health insurance vendors, places of worship, and campaigns funded through public-private collaboration. ...

Were asylums for severely, mentally ill individuals really so bad? Perhaps, these institutions needed to be updated etc., but not permanently closed. 

Unfortunately, the article is too much about entrenched interests of welfare workers and labor unions etc. What about the lobby of homeless individuals with mental disorders?

Ms. Perera also claims that "fiscal measures" and "budgetary pressures" contributed to the deinstitutionalisation of the 1970s/1980s. I am not sure about the relevance.

Damit wird der rosafarbene Papierausdruck nach und nach ersetzt. Ein Jahr später, Anfang 2025, bekommen nach fast 20 Jahren Vorlauf alle gesetzlich Versicherten eine elektronische Patientenakte (ePA) ausgestellt, es sei denn, sie widersprechen aktiv („Opt-out“). Privatversicherungen können die elektronische Patientenakte ebenfalls ausstellen. ..."

E-Rezept und E-Patientenakte für alle von Bundestag beschlossen Das Rezept auf dem Smartphone und digitalisierte Befunde und Laborwerte – das soll bald für alle Bürger in Deutschland verfügbar werden. Am Donnerstag hat der Bundestag die dafür notwendigen Gesetze beschlossen.
